CDC Worksite Health Score Card
The CDC Worksite Health ScoreCard (HSC) is a FREE online tool designed to help employers assess and improve health promotion programs in their worksites. The HSC has been evaluated and determined to be a reliable tool for employers to use in assessing health promotion programs aimed at improving lifestyle risks and preventing chronic disease among employees. The HSC helps employers identify gaps in their health promotion efforts and prioritize high-impact strategies for health promotion at their worksites.

This tool aids employers in assessing worksite health promotion by asking YES/NO questions across 16 health domains:
- organizational supports
- tobacco control
- physical activity
- weight management
- stress management
- high blood pressure
- high cholesterol
- signs and symptoms of heart attack and stroke
- lactation support
- occupational health and safety
- vaccine-preventable diseases
- community resources
